Outline of Case Management Services
Emerge Behavioral Health provides support to children, families and caregivers by:
- participating in client staffing to ensure services are integrative and client receives the most relevant care and services reflected in the Individualized Service Plan.
- facilitating treatment team reviews with the client, therapist, and other members of a treatment team including family members.
- monitoring client’s progress and needs through regular discussion with the client, therapist, designated school or vocational personnel, family, and other facility staff members.
- making regular contact with client and parent or legal guardian,
- documenting case management activities in chronological notes. This documentation consistently refers to the targeted goals and objectives specified in the Individual Service Plan.
- providing other written status reports to facility or referral source as requested.
- staying abreast of current information in the field and resources available through training attendance as recommended by the supervisor.
