Outline of Case Management Services

Emerge Behavioral Health provides support to children, families, and caregivers by:

  • Participating in client staffing to ensure services are integrative and the client receives the most relevant care and services reflected in the Individualized Service Plan.
  • Facilitating treatment team reviews with the client, therapist, and other members of a treatment team including family members.
  • Monitoring the client’s progress and needs through regular discussion with the client, therapist, designated school or vocational personnel, family, and other facility staff members.
  • Making regular contact with client and parent or legal guardian,
  • Documenting case management activities in chronological notes. This documentation consistently refers to the targeted goals and objectives specified in the Individual Service Plan.
  • Providing other written status reports to the facility or referral source as requested.
  • Staying abreast of current information in the field and resources available through training attendance as recommended by the supervisor.
